Insurance Information

At Haven Dental, we strive to make dental care accessible and affordable. We work with a variety of insurance providers to help you maximize your benefits.

Insurance Providers:

The insurance providers we are currently in network with:

  • Delta Dental PPO

  • Cigna

  • Premier & DeltaCare

  • Bluecross LA

  • Sunlife/DHA

  • Aetna (not Medicare)

  • UHC

  • Tricare

  • United Concordia​

If you are “Out-of-Network,” it simply means that if there is a difference between OUR fee and the Allowable Fee set by your insurance, you are responsible for the difference. Our fees are based on customary rates for our area. For most patients using their Out-Of-Network benefits, for Preventive and Diagnostic Services there will often be either a $0 or very minimal out-of-pocket cost. A lot of our patients have out-of-pocket costs between $20 and $40, but still prefer to come to us due our exceptional service.

Insurance Verification:

For accurate billing, we will verify your insurance coverage before your appointment. Please provide us with your insurance details, including the policy number and group number, at the time of your visit.

Coverage Details:

Coverage varies by plan. We recommend contacting your insurance provider to understand your benefits, including co-payments, deductibles, and coverage limits. Our team can assist with verifying your benefits and answering questions about your coverage.

Filing Claims:

Our office will file insurance claims on your behalf. We will work with your insurance company to ensure that claims are processed promptly and accurately.

Out-of-Pocket Costs:

You may be responsible for any co-payments, deductibles, or non-covered services. We will provide an estimate of out-of-pocket costs before your treatment begins. Payment is due at the time of service unless other arrangements are made.

Accepted Payment Methods:

We accept various payment methods, including:

  • Cash

  • Credit/Debit Cards (Visa, MasterCard, American Express, Discover)

  • Personal Checks (with valid ID)

  • Flexible Spending Accounts (FSA) and Health Savings Accounts (HSA)

Payment Plans and Financing:

For larger treatments or procedures, we offer financing options through third-party providers. 

  • Cherry financing-our most popular option with customized interest rates

  • Care Credit - we offer 6 months interest free or 36 months at 18.9%. â€‹

Payment Policy:

Payment is due at the time of service, unless other arrangements are made. For questions about billing or to discuss payment options, please contact our billing department.

If you have questions about your bill or need assistance with understanding charges, our billing team is here to help. Contact us at 985-624-8268 for support.

Refunds:

In the event of an overpayment or adjustment, we will issue a refund according to our office policy. Refunds will be processed to the original payment method used.
